When I entered the Violet Hold in Dalaran, Khadgar didn't appear. I disconnected and re-entered the game, but I found myself in the initial Demon Hunter quests. I completed the first quest, but now I have no more quests available. After investigating a bit more, I found that I'm missing some quests, starting with "The Escape" (ID=38672), which is the first quest in the missing quest chain. However, this is part of a scenario.
